Before your first appointment with one of our physicians, you may want to complete the medical history form below. We also request that you obtain medical records from your other health care providers and bring them with you if possible.

Directions and parking information to the Otolaryngology Head and Neck Surgery Department at Emory University:

Exit Interstate 85 at Clairmont Road (#32). Turn left onto Clairmont if traveling southbound on I-85; turn right if traveling northbound on I-85. Drive approximately four miles on Clairmont Road. Turn right onto North Decatur Road. Go approximately two miles on North Decatur Road, and turn right onto Clifton Road. The Emory Clinic Building A and B and valet parking are 100 yards on the right.
